Medicine is not a simple occupation. Arguably, it has similarities with religion. Therefore, the priesthood-like prestige will attract a type of determined and capable youth, as well as a good portion of doctor's progeny, regardless of the manner they are selected and tested.

This mindset persists as they age. You will find many MDs in healthcare management or epidemiological research, who'd point to their degrees even if they stopped practicing many years ago.

Moreover, the various standardized tests pale into irrelevance, that is, are downgraded to second-tier decision factors, or fully ignored, when bishops select like-minded deacons and priests. Sorry, I meant medical instructors and residents.

Conversely, you could have promised the largest yacht to Einstein of Feynman, and they'd still forgo medicine.

All in all, you can't change this priesthood-like guild by changing entry rules. Germany does need to cast a wider net, but solely because today it doesn't train enough of its citizens, after flight to Switzerland is taken into account.

There is also the option of delegating trivial tasks, such as i.m. mRNA shots, to non-doctors, unless that was a religious ritual.
